Youth Application: Required for any youth that are new to Cub Scouts.
Adult Application: Required for any adult that will have an active leadership role with the Pack and is new to Cub Scouts.
Medical Forms A, B1 and B2: Required for any adult or scout that will be attending Cub Scout Summer Camp.
Release Form: Required for any youth, adult, or family members that will be attending a Scouting event.
Family Information Form: We ask all families to complete our Family Information Form. We’re looking to confirm all parent contact information, ask about allergies or other medical conditions we should know about, and inquire about any skills/knowledge/access you may have that can be shared with our scouts (i.e., yoga instructor, police office, gardener).
