Pack 726 Release Form

Each parent/guardian must submit a form granting release.

I hereby grant permission to Cub Scout Pack 726 (of 2800 NW 153^rd Ave. Beaverton, OR 97229), its committee and adult leaders to use any images, photographs, videos, artwork, testimonials or voice recordings whether a hard copy or electronic files of the persons listed below for any legal use, including but not limited to the Pack’s: website, social media, news articles, flyers or in any other publications and/or promotional material.

I understand that these images, photographs, videos, artwork, testimonials or voice recordings can be viewed/heard by anyone in the world. Cub Scout Pack 726 will not list the anyone’s full name with any use of the images, photographs, videos, artwork, testimonials or voice recordings.

Furthermore, I understand that no royalty, fee or other compensation shall become payable to me or my family by reason of such use. This permission is good for the length of Pack 726’s tenure and can be revoked with a written note to the Pack Leadership Committee.

I am the parent or legal guardian of the following child(ren) under 18 years of age, and I give permission for their images, photographs, videos, artwork, testimonials or voice recordings to be published.
